Initial Application for CME approval - Directly Provided

Complete this Initial Application for consideration of internal approval of a CME activity with the Massachusetts Medical Society (MMS).  A separate planning document will be required for each activity.

Your application will be reviewed within 5-7 days of its receipt.  You may be contacted for additional information.  After review, you will be informed of the decision and, if accepted, you will receive the activity planning documents from the Education Compliance Office, which begins the formal process of submitting an activity for review by the MMS Committee on Sponsored Programs.
